About Us
Angels Care Home Health is an agency that provides Home and Community-Based Services (HCBS) and Personal Care Assistance (PCA).
The HCBS program services are person-centered care delivered in the home and community. The services offered by HCBS program addresses the needs of people with functional limitations who need assistance with everyday activities. The program enables people to stay in their homes rather than moving to a facility for care.
Our staff is licensed and trained appropriately to serve clients in their homes. They have the commitment and dedication to providing care services that our clients deserve. Furthermore, our agency has many kinds of programs and types of care through the HCBS. The HCBS standards under Minnesota Statutes, Chapter 245D, were part of a larger HCBS Waiver Provider Standards initiative to improve the dignity, health, and independence of the people we serve.
Our Care Assistance (PCA) services help a person with day-to-day activities in their home and community. PCA Choice is also another option that allows the recipient to decide which direct care staff will provide the services.
Our essential support services provide the level of assistance, supervision, and care that is necessary to ensure the health and safety of the person.
