Angels Care provides Home and Community-Based Services (HCBS). Our agency has many kinds of programs and types of HCBS.
HCBS provide services to persons with age 65 and older and person disabilities in Minnesota, subject to the standards of Minnesota Statutes, Chapter 245A and Minnesota Statutes, Chapter 245D.
We also have Community First Services and Supports (CFSS) and Personal Care Assistance (PCA) services that provide assistance and support for persons with disabilities, living independently in the community. The services include the elderly and others with special health care needs.
CFSS services are provided in the recipient’s home or in the community when normal life activities take him or her outside the home.
Our agency has well-trained health care professionals who provide the compassion that our clients need.
We also have Housing Stabilization Services (HSS). HSS is a new Minnesota Medical Assistance benefit to help people with disabilities, including mental illness and substance use disorder, and seniors find and keep housing.
